Marshall Islands are simply beautiful, 00:00:09.90\00:00:12.21 stunning, really, 00:00:12.24\00:00:13.61 750,000 square miles of deep blue Pacific Ocean. 00:00:13.64\00:00:18.78 But 1225 islands comprise the whole area, 00:00:18.81\00:00:23.62 was actually 29 atolls. 00:00:23.65\00:00:25.32 An atoll is an area that has a lagoon. 00:00:25.35\00:00:28.59 But all the other islands like you see in the distance 00:00:28.62\00:00:31.23 are dotted everywhere. 00:00:31.26\00:00:33.93 Unique, absolutely stunning and beautiful. 00:00:33.96\00:00:36.70 I've heard from so many people about the good work 00:01:10.10\00:01:12.07 the wellness center is doing in the neighborhoods. 00:01:12.10\00:01:14.14 I wanted to check it out for myself. 00:01:14.17\00:01:15.67 So let's see what's going on. 00:01:15.70\00:01:20.08 Tanner, is this pretty typical neighborhood? 00:01:20.11\00:01:23.11 Yes, it is. 00:01:23.14\00:01:24.48 Jim, this is a very typical neighborhood 00:01:24.51\00:01:26.25 you'll see here on Majuro with families 00:01:26.28\00:01:29.05 being pretty close knit, you know. 00:01:29.08\00:01:31.45 They'll actually have most of the family members, 00:01:31.49\00:01:34.26 you know, cousins, aunts, uncles in their little area. 00:01:34.29\00:01:37.96 Yeah. Okay. 00:01:37.99\00:01:39.66 And, kind of, is this a little typical 00:01:39.69\00:01:42.13 of the kind of a style or the quality of home 00:01:42.16\00:01:44.97 in the area too? 00:01:45.00\00:01:46.33 It is actually. 00:01:46.37\00:01:47.70 You usually see a lot of laundry, 00:01:47.74\00:01:49.07 of course, like you see here hanging outside. 00:01:49.10\00:01:51.27 A lot of times, they'll have the cooking done separately 00:01:51.31\00:01:54.68 from the inside of the house 00:01:54.71\00:01:56.38 where they'll just mainly have a flat bed, or mattress, 00:01:56.41\00:02:00.08 or sometimes just, 00:02:00.12\00:02:01.98 you know, sleeping on the dirt floor type of deal. 00:02:02.02\00:02:04.59 Now you've been organizing teams 00:02:04.62\00:02:06.19 and train them to go out in neighborhoods like this? 00:02:06.22\00:02:08.46 What are they doing? 00:02:08.49\00:02:09.82 So this program we started was with Ministry of Health 00:02:09.86\00:02:14.83 here in Majuro. 00:02:14.86\00:02:16.46 And they sent out teams already to see patients. 00:02:16.50\00:02:20.34 So we said, "Let's partner up with you guys. 00:02:20.37\00:02:23.00 And we'll send out our team of community health workers 00:02:23.04\00:02:26.21 to see the patients 00:02:26.24\00:02:27.88 and teach them lifestyle change. 00:02:27.91\00:02:30.08 And let's see if that doesn't help them 00:02:30.11\00:02:32.65 lower their blood sugar and their blood pressure, 00:02:32.68\00:02:35.82 help the wounds that they have on their feet, 00:02:35.85\00:02:37.85 a lot of times heal faster." 00:02:37.89\00:02:40.89 And right now, the program 00:02:40.92\00:02:42.59 has about four community health workers 00:02:42.62\00:02:45.93 that operate out of the wellness center 00:02:45.96\00:02:47.30 here in Majuro. 00:02:47.33\00:02:48.66 And then we see about 50 patients. 00:02:48.70\00:02:53.23 So tell me the success of this program to this point? 00:02:53.27\00:02:55.67 Right. 00:02:55.70\00:02:57.04 So the success so far we've seen 00:02:57.07\00:02:59.67 at least two people like 00:02:59.71\00:03:01.68 you see the woman in the background here 00:03:01.71\00:03:04.18 had a very bad wound on her heels. 00:03:04.21\00:03:06.61 I saw at her, it looked absolutely terrible. 00:03:06.65\00:03:08.45 Yeah. And this is from diabetes? 00:03:08.48\00:03:10.49 Exactly. 00:03:10.52\00:03:11.85 A lot of times, diabetics can't feel 00:03:11.89\00:03:14.62 when they get cuts on their feet. 00:03:14.66\00:03:16.06 It's called diabetic neuropathy in their feet. 00:03:16.09\00:03:19.03 So they don't feel that 00:03:19.06\00:03:20.40 and out here 00:03:20.43\00:03:21.76 things get infected very quickly. 00:03:21.80\00:03:23.63 So they got infected, 00:03:23.67\00:03:25.27 they don't know how to treat it. 00:03:25.30\00:03:26.90 They try maybe traditional medicine first. 00:03:26.94\00:03:29.64 That doesn't work. 00:03:29.67\00:03:31.01 So a lot of times by the time they come to the hospital, 00:03:31.04\00:03:34.51 it's too late for antibiotics to save the foot. 00:03:34.54\00:03:37.98 And the doctors have no choice but to amputate. 00:03:38.01\00:03:40.28 Wow. 00:03:40.32\00:03:41.65 So you're really doing intervention 00:03:41.68\00:03:44.65 in the neighborhoods before it gets to that point? 00:03:44.69\00:03:47.16 Exactly. That's what we wanna do. 00:03:47.19\00:03:48.86 We wanna get it before it gets bad, 00:03:48.89\00:03:51.49 where they have no choice but to chop off the leg. 00:03:51.53\00:03:54.20 The health workers we have went through a training course 00:03:54.23\00:03:56.97 with Secretary of the Pacific Community 00:03:57.00\00:04:00.27 where they were trained in how to help manage diabetes, 00:04:00.30\00:04:04.77 do self management goals with the patients. 00:04:04.81\00:04:07.94 And then each patient gets their blood sugar checked. 00:04:07.98\00:04:12.01 Blood pressure checked, if they have two legs, 00:04:12.05\00:04:15.82 they can get walking shoes to walk. 00:04:15.85\00:04:18.65 And then we'll also help them find wheelchairs 00:04:18.69\00:04:21.76 if they need wheelchairs. 00:04:21.79\00:04:23.46 Which is a real tough thing on this island. 00:04:23.49\00:04:25.19 Very tough thing. Yeah. 00:04:25.23\00:04:26.56 Very hard commodity to find. 00:04:26.59\00:04:29.10 And we'll also encourage them to get active 00:04:29.13\00:04:32.23 and exercise any way they can. 00:04:32.27\00:04:34.27 Like a lot of the patients we see at home 00:04:34.30\00:04:36.60 have had amputations already. 00:04:36.64\00:04:39.41 And so, we'll give them the exercise bands 00:04:39.44\00:04:41.68 that they can do and teach them how to do 00:04:41.71\00:04:43.71 the stretching exercises with those. 00:04:43.75\00:04:46.51 And what we've seen is over time, 00:04:46.55\00:04:48.75 their health improves. 00:04:48.78\00:04:50.39 You know, a lot of them when we start out, 00:04:50.42\00:04:51.95 they'll complain. 00:04:51.99\00:04:53.32 "Oh, you know, my body aches all the time. 00:04:53.36\00:04:55.99 My vision is blurred. 00:04:56.02\00:04:57.86 I'm dizzy all the time." 00:04:57.89\00:04:59.76 And within just a few weeks, 00:04:59.79\00:05:01.66 once they get their blood sugar under control, 00:05:01.70\00:05:06.13 those symptoms go away. 00:05:06.17\00:05:07.87 Yeah, I'm just guessing you're the nurse. 00:05:07.90\00:05:10.07 But just by you being around 00:05:10.11\00:05:13.68 and coming to the home doing those regular visits, 00:05:13.71\00:05:15.61 you're offering hope. 00:05:15.64\00:05:16.98 And that helps them also. 00:05:17.01\00:05:18.35 Yeah, you can see the smile come back to their face, 00:05:18.38\00:05:21.58 when they start feeling better, and they have more energy now. 00:05:21.62\00:05:25.12 And they start to see on their machine, you know, 00:05:25.15\00:05:27.26 "Hey, my blood sugar was 500, and now it's 300, 00:05:27.29\00:05:30.96 and then 200, and then 100." 00:05:30.99\00:05:32.99 And you just see that smile get bigger and bigger, 00:05:33.03\00:05:35.96 you know, and it's a real joy to see that. 00:05:36.00\00:05:39.07 And we have that happening all over the island 00:05:39.10\00:05:40.90 with these people. 00:05:40.94\00:05:42.27 Now their sugars are coming down, 00:05:42.30\00:05:44.57 you know, we check A1C levels on them. 00:05:44.61\00:05:47.74 A1C, translate that for me? 00:05:47.78\00:05:49.24 Okay, A1C is a blood test that checks 00:05:49.28\00:05:53.15 how well your blood sugar does over 90 days. 00:05:53.18\00:05:57.29 So you and me would be about 4%, 00:05:57.32\00:06:01.09 diabetic is usually above seven. 00:06:01.12\00:06:04.19 So a lot of the people 00:06:04.23\00:06:05.56 that we see here are 11, 13, 14. 00:06:05.59\00:06:08.46 And they're coming down to 8, 7, 6. 00:06:08.50\00:06:12.13 Now you also go right to the core of the problem 00:06:12.17\00:06:14.84 and changing their diet, 00:06:14.87\00:06:16.20 which is what originally caused 00:06:16.24\00:06:17.57 a lot of these problems, is that correct? 00:06:17.61\00:06:18.94 Right. 00:06:18.97\00:06:20.31 One of the keys to this program 00:06:20.34\00:06:21.68 is that we come to them 00:06:21.71\00:06:23.51 and meet them where they're at. 00:06:23.55\00:06:25.51 And we don't bring any food in there with us. 00:06:25.55\00:06:29.92 We teach them how to cook with the food 00:06:29.95\00:06:32.19 that they have at home. 00:06:32.22\00:06:33.79 Because if it's not familiar to them, 00:06:33.82\00:06:36.26 they're not going to eat it. 00:06:36.29\00:06:38.59 So we teach them, you know, if you're gonna eat chicken, 00:06:38.63\00:06:41.13 for example, a lot of people love to fry chicken, 00:06:41.16\00:06:44.67 and they deep fry it in a ton of oil. 00:06:44.70\00:06:47.60 So we teach them, "Okay, you can boil that, 00:06:47.64\00:06:50.81 and take the skin off the chicken, 00:06:50.84\00:06:53.21 reduce the fat, " 00:06:53.24\00:06:54.84 just simple little things like that, 00:06:54.88\00:06:56.78 that can make a ton of difference 00:06:56.81\00:06:58.28 in a person's diet. 00:06:58.31\00:06:59.65 But even that is a shocker to a lot of people 00:06:59.68\00:07:01.52 because they like that skin, they like all that flavor. 00:07:01.55\00:07:04.52 So how do you help them transition? 00:07:04.55\00:07:07.62 One of the things we do 00:07:07.66\00:07:09.19 is we encourage them to start garden. 00:07:09.22\00:07:12.79 So part of the program 00:07:12.83\00:07:14.16 is we bring each family to garden boxes, 00:07:14.20\00:07:18.77 and we supply them with the soil and the plants. 00:07:18.80\00:07:21.40 And we make sure that they understand, 00:07:21.44\00:07:23.20 you know, this is theirs now, 00:07:23.24\00:07:24.57 this is something that they can use 00:07:24.61\00:07:27.68 to supplement, you know, 00:07:27.71\00:07:29.74 the high costs that you see in the stores out here. 00:07:29.78\00:07:32.21 When you come out here, you're shocked 00:07:32.25\00:07:34.82 because a cucumber can cost you $5. 00:07:34.85\00:07:37.95 You know, so we have them growing easy stuff. 00:07:37.99\00:07:42.99 Spinach, kangkong 00:07:43.02\00:07:44.89 which is a Spinach type of deal 00:07:44.93\00:07:47.03 where you can cut it down and it will re-grow. 00:07:47.06\00:07:49.83 And they use that and we tell them, 00:07:49.86\00:07:51.30 you know, put it into your dish with your rice. 00:07:51.33\00:07:55.47 Now tell me about your future plans. 00:07:55.50\00:07:58.34 I've got a feeling you just don't wanna leave this 00:07:58.37\00:08:00.21 right where it is? 00:08:00.24\00:08:01.58 No, we don't at all. 00:08:01.61\00:08:02.94 This is a two year kind of pilot program 00:08:02.98\00:08:07.15 that we're doing here. 00:08:07.18\00:08:08.52 And we're hoping 00:08:08.55\00:08:09.88 that in conjunction with the hospital, 00:08:09.92\00:08:11.95 we're looking to form an actual home health team 00:08:11.99\00:08:16.29 that would reach island wide, 00:08:16.32\00:08:18.39 and then eventually 00:08:18.43\00:08:19.86 out to the outer islands as well. 00:08:19.89\00:08:21.53 Now I was talking just the other day 00:08:21.56\00:08:24.17 with a Senator 00:08:24.20\00:08:25.53 who as of now's Minister of Health, 00:08:25.57\00:08:27.54 he just had tremendous praise for the wellness center 00:08:27.57\00:08:30.21 and what you're doing because he said, 00:08:30.24\00:08:31.57 "We can't do it all." 00:08:31.61\00:08:32.97 So that's what you're looking for 00:08:33.01\00:08:34.34 is these partnerships to help. 00:08:34.38\00:08:36.91 Right. 00:08:36.95\00:08:38.28 You have to partner with people. 00:08:38.31\00:08:40.02 Out here, you can't 00:08:40.05\00:08:41.38 like you said you can't do it by yourself. 00:08:41.42\00:08:43.08 There's no way you can do it. 00:08:43.12\00:08:44.72 There's simply too much need. 00:08:44.75\00:08:46.92 So we've gone and developed over the last 10, 12 years, 00:08:46.96\00:08:51.93 many different relationships out here on the island, 00:08:51.96\00:08:55.00 with NGOs 00:08:55.03\00:08:56.36 and with government organizations 00:08:56.40\00:08:57.90 so that we have those connections now 00:08:57.93\00:09:00.10 that we can say, "Okay, 00:09:00.14\00:09:01.47 let's all sit down and talk about 00:09:01.50\00:09:03.57 how we can solve this problem together." 00:09:03.61\00:09:05.54 Give me a five year plan 00:09:05.57\00:09:07.24 or a dream actually at the moment 00:09:07.28\00:09:09.21 maybe but give me a five year dream? 00:09:09.24\00:09:12.01 Five Year dream 00:09:12.05\00:09:13.48 out of this would be that each village, 00:09:13.52\00:09:17.02 each village here is called watos. 00:09:17.05\00:09:18.75 Say it again? Watos. 00:09:18.79\00:09:20.12 Watos. 00:09:20.16\00:09:21.49 Yeah, each village would have 00:09:21.52\00:09:22.86 its own community health workers 00:09:22.89\00:09:25.59 that would be in charge of taking care of their area. 00:09:25.63\00:09:29.90 And they would be trained in diabetes counseling, 00:09:29.93\00:09:33.34 basic education on for nutrition. 00:09:33.37\00:09:36.81 They be trained in how to do cooking with the patients, 00:09:36.84\00:09:39.97 and they be kind of the overseers 00:09:40.01\00:09:42.21 of the health for that community. 00:09:42.24\00:09:45.18 Now, you've probably if you've done 00:09:45.21\00:09:47.88 that five year kind of dream, 00:09:47.92\00:09:49.25 how many workers 00:09:49.28\00:09:50.62 are we talking about to fulfill that? 00:09:50.65\00:09:53.05 I think the early estimates put it at about 25 at least. 00:09:53.09\00:09:56.66 Twenty five. 00:23:19.06\00:23:20.40 Because Jesus was the creator. 00:23:20.43\00:23:22.16 I invite you today to behold Jesus as the creator. 00:23:22.20\00:23:26.20 And Jesus, we see Him 00:23:26.23\00:23:27.57 just before the time of the crucifixion, 00:23:27.60\00:23:30.07 He's in the Garden of Gethsemane. 00:23:30.11\00:23:32.94 He stayed under a tree, 00:23:32.97\00:23:34.31 He poured out His heart to the Father, 00:23:34.34\00:23:35.84 because as the God man, He'd taken on human form, 00:23:35.88\00:23:39.38 human life, and a young man in His early 30s, 00:23:39.41\00:23:41.68 He didn't want to die. 00:23:41.72\00:23:43.65 He really didn't want to die, says, 00:23:43.69\00:23:45.15 "Father, please, if this cup can pass from Me, 00:23:45.19\00:23:48.16 let it be so." 00:23:48.19\00:23:49.76 No word, nothing. 00:23:49.79\00:23:51.49 "Nevertheless, Father, not My will, 00:23:51.53\00:23:54.20 but Your will be done." 00:23:54.23\00:23:55.56 That happened three times, 00:23:55.60\00:23:57.20 "Not My will, but Your will be done." 00:23:57.23\00:24:00.24 Finally, He was completely resigned 00:24:00.27\00:24:02.10 to the fact that He would do the Father's will, 00:24:02.14\00:24:05.14 He chose to do the Father's will. 00:24:05.17\00:24:08.51 He was then captured, taken to Pilate. 00:24:08.54\00:24:10.55 Were back to that same Pilate that said, 00:24:10.58\00:24:12.15 "Behold the man." 00:24:12.18\00:24:14.15 And then they took Him to Calvary's Cross. 00:24:14.18\00:24:17.22 He laid down on that cross willingly 00:24:17.25\00:24:19.39 because that's why He came to planet earth 00:24:19.42\00:24:21.32 in order to die for you and me. 00:24:21.36\00:24:23.19 Laid there as they pounded those nails 00:24:23.22\00:24:25.13 into His hand, into His feet. 00:24:25.16\00:24:29.13 He was bleeding hanging there. 00:24:29.16\00:24:31.20 He looked at the crowd, said, "Father, forgive them. 00:24:31.23\00:24:35.37 They don't know what they're doing." 00:24:35.40\00:24:37.04 Truly, they didn't, 00:24:37.07\00:24:38.41 they were crucifying God Himself. 00:24:38.44\00:24:42.18 And then they took Him, placed Him in a tomb. 00:24:42.21\00:24:45.65 I've had the opportunity to be in that tomb, 00:24:45.68\00:24:49.38 to lay almost in that tomb. 00:24:49.42\00:24:52.72 And guess what?
