over time. 00:06:23.41\00:06:26.78 Now there's very few fishermen in comparison 00:06:26.82\00:06:29.15 to the worldwide fishing 00:06:29.18\00:06:30.89 that goes on China and Japan and many other the seashore and He called out to them, 00:19:24.46\00:19:28.23 "Cast your nets on the other side of the boat." 00:19:28.26\00:19:31.17 Well, it turned out that was the side 00:19:31.20\00:19:32.63 between He and the disciples. 00:19:32.67\00:19:36.94 They cast the nets in, after a little bit of argument 00:19:36.97\00:19:39.51 because they were seasoned fishermen. 00:19:39.54\00:19:40.94 They didn't want to listen to Jesus. 00:19:40.98\00:19:42.94 Cast the nets in, the catch was huge. 00:19:42.98\00:19:45.88 They pulled up this mighty catch. 00:19:45.91\00:19:47.55 Why? You can't compromise. 00:22:48.03\00:22:50.27 So tell us before the patient shows up, 00:22:50.30\00:22:51.87 what's going to be happening here today? 00:22:51.90\00:22:53.23 All right. 00:22:53.27\00:22:54.60 So we have an elderly patient 00:22:54.64\00:22:56.40 with unusual disease for this area. 00:22:56.44\00:22:59.37 It's called age related macular degeneration. 00:22:59.41\00:23:02.21 And this happens to the elderly. 00:23:02.24\00:23:04.18 And the average lifespan is not very long here. 00:23:04.21\00:23:08.32 I know in Majuro, I believe it's like 55. 00:23:08.35\00:23:10.85 So I'd imagine it's not too different here. 00:23:10.89\00:23:12.85 So this is a disease that happens after age 50. 00:23:12.89\00:23:15.72 I got to stop here 'cause the average age is 55. 00:23:15.76\00:23:19.03 That's it. I believe it is in Majuro. 00:23:19.06\00:23:21.16 Like now for here, I suspect it's a little longer but... 00:23:21.20\00:23:24.13 But somewhere in that neighborhood. 00:23:24.17\00:23:25.53 Yeah, which is scary. 00:23:25.57\00:23:26.90 So definitely a lot shorter than it is. 00:23:26.94\00:23:28.97 It is, like infant mortality is really high. 00:23:29.00\00:23:31.71 It tends to spike, you know, infant mortality is high. 00:23:31.74\00:23:34.48 And then there's one more point in your life if you make it 00:23:34.51\00:23:36.85 through the 20s, then you're very likely 00:23:36.88\00:23:38.45 to make it to your 50s. 00:23:38.48\00:23:40.05 Wow, okay. 00:23:40.08\00:23:41.42 So this person has that particular... 00:23:41.45\00:23:43.12 They do. 00:23:43.15\00:23:44.49 They have age related macular degeneration, 00:23:44.52\00:23:46.22 which in the US is the number one cause 00:23:46.25\00:23:47.92 of vision loss in our retired population. 00:23:47.96\00:23:51.29 Here, you know, there's not very many retired 00:23:51.33\00:23:53.63 elderly people, but we have one 00:23:53.66\00:23:55.60 and she has cataract in both eyes. 00:23:55.63\00:23:58.43 And she has advanced age related 00:23:58.47\00:24:00.14 macular degeneration. 00:24:00.17\00:24:01.80 There's two types, a dry type, which is usually more mild, 00:24:01.84\00:24:04.61 slow progressing. 00:24:04.64\00:24:05.97 And then a wet type. 00:24:06.01\00:24:07.71 And the wet type is when you have bleeding 00:24:07.74\00:24:09.38 in the eye and rapid vision loss is severe. 00:24:09.41\00:24:12.21 What it does it cause central vision loss 00:24:12.25\00:24:14.28 and can cause peripheral 00:24:14.32\00:24:15.72 and very rarely like 1% of the time. 00:24:15.75\00:24:18.05 And we have a one percenter with us today. 00:24:18.09\00:24:19.82 So her blood broke through the retina, 00:24:19.85\00:24:22.06 spread out from the center and is in outer edges. 00:24:22.09\00:24:24.79 So what we're going to do is take the cataract out, 00:24:24.83\00:24:27.36 and then take the blood out of the eye, 00:24:27.40\00:24:30.10 and then check the macula. 00:24:30.13\00:24:31.47 We haven't really seen it yet. 00:24:31.50\00:24:33.20 We're presuming that's the diagnosis 00:24:33.23\00:24:34.67 because we could see the other eye. 00:24:34.70\00:24:36.57 So this is the removal of the cataract 00:24:36.60\00:24:39.44 in removing the blood 00:24:39.47\00:24:40.81 is that one part of the surgery or... 00:24:40.84\00:24:42.61 That's two separate parts, yes. 00:24:42.64\00:24:44.31 So traditionally, cataract surgery 00:24:44.35\00:24:46.18 and then they'd come back weeks or months later, 00:24:46.21\00:24:48.82 and do the other eye. 00:24:48.85\00:24:50.19 But since she here only two weeks? 00:24:50.22\00:24:52.62 All in one day, yeah. Okay. 00:24:52.65\00:24:55.46 So we'll get the blood out of the way 00:24:55.49\00:24:57.46 and then we'll check the macula, 00:24:57.49\00:24:58.83 and it's very possible we'll have 00:24:58.86\00:25:00.20 some healthy tissue back there 00:25:00.23\00:25:02.06 and make a huge difference for the patient. 00:25:02.10\00:25:04.67 But it's more likely that they'll be a scar 00:25:04.70\00:25:07.27 in the center. 00:25:07.30\00:25:08.64 And will make a moderate difference for the patient 00:25:08.67\00:25:11.07 meaning we can bring back her peripheral vision, 00:25:11.11\00:25:13.27 but she'll still have a blind spot 00:25:13.31\00:25:14.64 in the very center, 00:25:14.68\00:25:16.01 but peripheral visions usually much you walk around ambulate, 00:25:16.04\00:25:19.18 so you don't need someone taking care of you 24/7. 00:25:19.21\00:25:21.25 You're here two weeks, of course, 00:25:24.02\00:25:25.35 this time unfortunately you got cut short 00:25:25.39\00:25:26.89 because there was an emergency on the plane. 00:25:26.92\00:25:28.36 You had to go back to Hawaii. Yes. 00:25:28.39\00:25:30.59 But so you lost a day. 00:25:30.63\00:25:32.36 But typically, how many patients can you do? 00:25:32.39\00:25:35.53 How many cataract surgeries can you do? 00:25:35.56\00:25:37.70 So on a mission trip? 00:25:37.73\00:25:39.97 Well, the cataract surgeons, I'm a retina surgeon, 00:25:40.00\00:25:42.30 but so the cataract surgeons, I believe they did 00:25:42.34\00:25:45.71 a little less than 200 cataract surgeries alone. 00:25:45.74\00:25:48.88 So over 200 retina also. Two hundred? 00:25:48.91\00:25:50.68 Yeah, we're two weeks.
